Please complete the form below to nominate an exceptional 911 professional for the Stellar Coin, a recognition given as part of the Illuminate 911 Excellence Awards. Note: Please submit only one nomination per form. If you wish to nominate multiple individuals, please complete a separate form for each nomination.

Evaluation Process:

• Review of specific incidents where the nominee demonstrated excellence.
• Nominations submitted by peers, supervisors, or Replay Evaluators.
• QA evaluations and supporting documentation of performance.

The Stellar Coin honors any 911 professionals within the United States who demonstrate outstanding overall performance, either through consistent excellence or during high-impact situations. Nominations are accepted year-round.

2. Consistently high QA scores across multiple evaluations.
3. Proactive problem-solving and effective decision-making under stress.
4. Notable contributions to team success during significant events or through ongoing performance.
5. Acts of professionalism, compassion, and skill that go above and beyond routine duties.
